Ethnobotanical uses and assessment of Anti-inflammatory, Antioxydant, and Antisickling Activities of Annona muricata L., 1753: A systematic Review and Meta-analysis

Annona muricata L. (Annonaceae) is widely used in traditional medicine to treat various inflammatory, metabolic and parasitic conditions, but scientific evidence is fragmentary. This systematic review and meta-analysis aimed to evaluate its ethnobotanical uses and its pharmacological properties, particularly its anti-inflammatory, antioxidant and anti-sickle cell activities. An exhaustive literature review (2010–2025) was conducted according to the PRISMA 2020 recommendations. Studies of ethnobotany, phytochemistry and pharmacology (in vitro, in silico and in vivo) were selected according to the PICOS criteria. Quantitative data were harmonised and analysed using random effects models (REML), and risk of bias was evaluated using appropriate tools (NOS, JBI, ROBINS-I, SYRCLE, RoB2). The results confirm significant and reproducible anti-inflammatory activity, attributed to flavonoids and polyphenols. The antioxidant activity is notable, but it is highly heterogeneous, reflecting the influence of solvents and evaluation methods. The anticycling activity is homogeneous and related to acetogennines and polyphenols, which inhibit the polymerisation of haemoglobin SS. In conclusion, A. muricata exhibits promising therapeutic properties. However, the robustness of the conclusions is limited by methodological heterogeneity, publication bias and the absence of clinical trials. Standardisation of extracts, validation in vivo and rigorous clinical trials are essential to confirm its pharmacological and therapeutic interest.
